Do males even have estrogen?

It’s common knowledge that testosterone is the male reproductive hormone, and estrogen is the female reproductive hormone. Estrogen plays a major role in breast development and regulating the menstrual cycle.

However, both testosterone and estrogen are present in every human body. It’s just that biological males have higher amounts of testosterone while biological females have higher amounts of estrogen.

In biological males, a particular form of estrogen, called estradiol, plays a role in the development of sexual characteristics and is even involved in the production of sperm. 

When transitioning from male to female (MTF), the goal is to increase the body’s levels of estradiol from 10–40 pg/mL to 100–200 pg/mL. This will feminize your body by helping you:

  • Redistribute fat to your hips and waist

Step 1 – Estrogen Levels in a Prepubescent Male

Before puberty, male estrogen levels are almost undetectable. Before we get into the details, it’s important to understand the different types of estrogen and what they do.

In males, there are two main types of estrogen.

  • Estradiol: The main form of estrogen in males whose function is to regulate sex drive, help in sperm production, and regulate testicular function.
  • Estrone: Helps in sperm production.

In a prepubescent male, the level of estrone in the body will only be as high as 16 pg/mL, and estradiol only 13 pg/mL. There are two reasons for this.

  1. A prepubescent male has yet to develop secondary sexual characteristics. Since estrogen regulates these characteristics, its presence is unnecessary in a young male. 
  2. The main source of estrogen in males comes from the conversion of testosterone to estradiol. The enzyme aromatase is crucial in this process. Since young males have low testosterone levels (less than 20 nanograms per deciliter), it’s impossible for estrogen levels to be high in their systems.

Step 2 – Estrogen Levels in a Pubescent Male

As puberty approaches, usually between the ages of 10 and 14, male estrogen levels start to increase, as you can see in the table below.

Although they can still be undetectable in some males, estradiol and estrone levels can go as high as 40 pg/mL and 60 pg/mL respectively. During puberty, estrogen in the male body is produced by fat cells, adrenal glands, and the testes (although indirectly).

To explain, the male body can convert testosterone into estradiol using an enzyme called aromatase. This enzyme can be found in the adrenal glands, testicles, the brain, and in fatty tissue.

While male estrogen levels directly correlate to a person’s age, weight, pubertal stage, and testosterone levels, testosterone has the greatest influence of all.

If testosterone levels are low, the body will lack the necessary raw materials to produce estradiol, leading to symptoms of hormone imbalance such as hot flashes and moodiness.

Step 3 – Estrogen Levels When Transitioning From Male to Female

One of the first things to tick off your MTF transition checklist is to raise your estrogen levels to 200 pg/mL and decrease your testosterone levels to the range that’s normal for a biological female (30–100 ng/dL).

There are two ways to do this.

The first is through hormone replacement therapy (HRT). This involves taking hormone blockers to suppress the production of testosterone, then taking estrogen to trigger the onset of feminine traits.

You can take estrogen in the form of pills, injections, cream, or a patch. As your estrogen levels increase, you’ll begin to experience changes like:

  • Decreased muscle mass
  • Thinner body hair
  • Softer skin
  • Mood swings
  • Enlarged breast tissue

These are all signs of positive progress. Still, HRT may cause you to experience additional side effects, such as:

  • Weight gain
  • Infertility
  • Increased risk of blood clots

Because of the potential for less desirable side effects, you’ll need to go for checkups every three months during the first year of treatment, and then every six to 12 months. 

Visits to the doctor are necessary to ensure your estradiol and testosterone levels stay within female norms. Your physician will also monitor your prolactin levels.

Prolactin is a hormone that balances estradiol, and its function is to ensure your estrogen levels remain optimal so that you see results faster.

Step 4 – Estrogen Levels in an Adult Male

In an adult male, estrone levels fall between 10 and 60 pg/mL while estradiol levels commonly range from 10 to 40 pg/mL.

When estrogen levels exceed these ranges, it can lead to infertility and erectile dysfunction. Other possible symptoms of high estrogen in males include:

  • Low sex drive
  • Loss of bone density
  • Hot flashes
  • Tiredness

  • Shrinking muscle mass

When estrogen levels fall below 10 pg/mL, it can lead to many of the same symptoms as high estrogen including low sex drive, moodiness, and recurring night sweats.

Keep in mind that while these male estrogen levels are standard across the majority of the population, they’re not true for everyone. Body chemistry varies from person to person, and testing methods change with time.

This means that a person could have normal levels but still experience symptoms of low estrogen. Similarly, a person may have abnormal estrogen levels without experiencing any symptoms. It all depends on the anatomy of your body.
